Health Outreach Coordinator – Full-Time

Location: Oakland Park, FL 33334
Schedule: Rotating office and outreach schedule (Tues-Fri 8 am–5:30 pm, Sat 8 am–1 pm for in-office; Tues 8 am–5:30 pm, Wed-Sat 2 pm–11 pm for outreach)

Job Overview:
We are seeking a passionate and dedicated Health Outreach Coordinator to join a growing team focused on community health and well-being. This role will involve engaging with diverse communities to promote health education, manage outreach programs, and ensure effective implementation of health services. The ideal candidate will have expertise in public health, social work, and project management, with a strong commitment to improving access to healthcare.

Key Responsibilities:

  • Community Health Outreach & Education:
    Lead and support community health initiatives by attending and organizing outreach events, conducting health screenings, and distributing essential health resources, ensuring HIPAA compliance.
  • Program Coordination & Implementation:
    Manage the Mobile Testing Unit as needed, ensuring smooth and efficient operation. Deliver health education and services to underserved populations, ensuring proper documentation of activities.
  • Patient Support & Retention:
    Serve as the first point of contact for patients, particularly in HIV/AIDS care, providing guidance and counseling on treatment adherence and health management. Support clients in maintaining engagement with healthcare services.
  • Health Advocacy & Coordination:
    Advocate for patient care needs, collaborating with healthcare providers and insurance programs to ensure patients have access to the necessary resources and treatment. Assist clients in navigating retention models and healthcare programs.
  • Team Collaboration & Reporting:
    Participate in team meetings, collaborate with other staff members, and ensure all outreach activities are documented and reported in a timely manner.

Qualifications & Skills:

  • High school diploma or GED (Required)
  • Bachelor’s degree in social science or a related field (Preferred)
  • At least 6 months of case management or social work experience (Preferred)
  • Valid Driver’s License (Required)
  • Strong leadership, communication, and organizational skills
  • Ability to work evenings and weekends as required
